The Emergency Healthcare Record (EHR) Card survey is seeking to evaluate the effectiveness of the EHR card project which was developed by Cork North Community Work Department/HSE Social Inclusion in partnership with Refugees. The EHR card is a wallet sized healthcare passport which helps communicate personal and medical information with health professionals in an emergency. Whilst many migrants and Refugees across Cork and Kerry have availed of these cards they have also proven very effective and popular with other cohorts experiencing communication difficulties such as patients who have cognitive impairment. The cards have been piloted in emergency/acute departments in CUH, MUH, UHK and Mallow General Hospital in addition to being widely distributed in communities. The survey includes 64 simple questions and takes less than 15 minutes to complete. It is available in English, Ukrainian and Arabic, however if another language is required feel free to contact me and I will assist. Participants who complete the survey in full will be entered in a draw to have a chance at winning a €100 one for all voucher.
Anybody can fill in this survey including staff and service users. It is also not necessary to be a card holder in order to submit a response.
